The hose and fan draws fresh air in from the outside and yes, you do need it running whilst the engine is running. Normally it is connected to the electrical on/off switch. so that the fan stops when the engine is off. If it does not turn off when the on/off switch is off, it will run your starter battery down in a few hours.
